Signore Pietro was a merchant allied with the Medici family. This sculpture-portrait is considered a masterpiece and is a prominent example of meticulousness. Clinically, it is of interest that the "dysmorphic" ears are asymmetric. Notice that the right ear has a gap on the external rim (helix), and concurrently the scapha (boat like concavity) is excessively deep - see related images showing the ears in greater detail. 

Generally speaking, unless Signore Mariano had other "dysmorphic" signs or his genetic ancestors did, the noted ear (auricle, pinna) dysmorphology is unlikely to have impacted his "health" with the exception, perhaps, his self-esteem.
