A large tongue (macroglossia), among other causes, may be due to an angioma (vascular anomaly), lack of thyroid function (cretinism), etc. In this instance, the infant is large (nearly obese or macro-somatic), which suggests the possibility of a high cancer risk syndrome (see our companion "Clinical Eye Openers" website and focus on macroglossia).
